Qualifications & Registration To qualify, the applicant must be a full time student at an accredited high school. Applicants must have participated in the prom activity and duly crowned as King, Queen, Prince or Princess by the school’s Prom Committee. To qualify, the student must (a) complete the ONLINE REGISTRATION FORM (b) Mail/fax to us a completed, signed and stamped copy of the CERTIFICATION APPLICATION FORM along with a clear, bright passport size photo. The Certification Application Form may be downloaded from our website. The form must be signed by the school’s Principal, Vice Principal or the teacher in-charge of student affairs or social affairs. The following are instructions for sending a copy of your photo Two Methods of Sending Your Photo (1) By E-Mail(2) By U.S. Postal Mail IF SENDING BY U.S. Postal MAIL (1) Your photo must contain only your image (one image, not group or it will be rejected)(2) We would suggest a bright clear photo since we may reduce the size to meet our publication requirements (3) DO NOT STAPEL OR TAPE YOUR PHOTO TO THE APPLICATION (4) Please remember to enclose your CERTIFIED APPLICATION FORM) Mail Photo to:
